PARTNERSHIPS
Lucia Williamd works collaboratively with mental health services and all other related organisations to provide structured mental health outreach and recovery support across London.The service is designed to support individuals with complex or ongoing mental health needs through coordinated, community-based intervention.
Step 1 Initial Contact
Referrals can be made by professionals, services, family members, or individuals seeking support. Initial contact is used to understand presenting needs and determine suitability for support.
Step 2: Initial Assessment
A structured assessment is completed to identify needs, strengths, risks, and recovery goals. This ensures support is tailored and appropriate from the outset.
Step 3: Recovery Planning
A personalised recovery plan is developed collaboratively, focusing on achievable goals, independence, and improved wellbeing.
Step 4: Community-Based Outreach Support
Practical, community-based support is delivered to promote engagement, stability, and access to appropriate services.
Step 5: Multi-Agency Working
Where required, support is coordinated with NHS services, Community Mental Health Teams, local authorities, and other professionals to ensure joined-up care.
Step 6: Review and Progression
Regular reviews are carried out to monitor progress, adjust support where needed, and ensure outcomes remain relevant and achievable.
