At Ascend Pro Health, my mission is simple yet vital: to partner with mental health services to deliver structured, person-centred support for people with recurrent and enduring mental health difficulties and early intervention for those at risk of developing mental health issues and complex needs. As a dedicated CaseWorker , I provide support grounded in lived experience, collaboration, and positive community inclusion. My approach is built on clarity, accessibility, and transparency at every stage of service delivery from initial engagement through to recovery, comprehensive assessments, tailored recovery plans, referrals, and regular care reviews.
Through respectful partnerships, evidence-informed practice, and a deep understanding of each individual’s unique strengths and goals, I deliver support that is both practical and transformative.. Collaboration is central to my work. I work directly with mental health services and related organisations to ensure continuity of care. I hold a current, valid enhanced DBS check and am fully insured, providing peace of mind for clients and partners alike.
